F.3.A.R หรือ FEAR 3 ภาคนี้จะเน้นระบบการเล่นแบบ Co-Op ซึ่งก็คือการเล่น 2 คนช่วยกันทำภารกิจผ่านเนื้อเรื่องของเกมนั่นเอง และตัวเอกใน FEAR 3 นี้ก็ไม่ใช่ใครที่ไหน มันคือลูกชายสุดแสบสองคนของ Alma ในภาคเก่า นั่นคือ Point Man และ Paxton Fettel นั่นเอง โดยผู้เล่นจะเลือกเล่นเป็นตัวไหนก็ได้ระหว่าง Point Man และ Paxton Fettel โดยทั้งสองตัวนั้นจะมีความสามารถที่แตกต่างกันไป ซึ่ง Point Man เป็นตัวที่มีพลังมากในสไตล์ super soldier ส่วน Paxton Fettel จะมีความสามารถในการใช้พลังจิต telekinetic เกม F.E.A.R. 3 มีโหมดมัลติเพลเยอร์ถึง 4 โหมดที่เป็นพิเศษ ไม่เหมือนกับเกมมัลติเพลเยอร์อื่นๆที่มีโหมด Deathmatch, Team Deathmatch หรือ Capture the Flag โดยระบุว่าล้าสมัยไปแล้ว ซึ่งโหมดทั้ง 4 ได้แก่โหมด Contractions, Soul King, Soul Survivor,และ Fucking Run อีกทั้งสามารถหยิบขึ้นเล่นออนไลน์เมื่อไรก็ได้ และโหมดดังกล่าวมีให้เห็นเฉพาะเกม F.E.A.R. 3 เท่านั้น
